Please complete this form for a student who is applying for admission to our arts magnet program.  Students should supply you with a stamped envelope with our address on it.  You will mail the form directly to us.  Recommendations forms are not shared with applicants.   Ask a teacher who knows you well and can speak to your interest/ability in art and your work ethic. Family members should not complete this form!  The back of this sheet may be used if needed.
